Permission

By paying and submitting this form, I give permission for my child to attend the aforementioned Central Ohio Youth for Christ trip or event. I hereby grant permission to Central Ohio Youth for Christ to secure emergency medical treatment for my child and permission to the attending physician, medical facility and staff to render medical care in the event of illness or injury. I certify that I am of lawful age and competent to sign this release and have done so voluntarily. I understand that this document constitutes a full and complete waiver of all possible claims for any act or omission, including claims for negligence regarding injury or property damages arising out of my child's participation in the trip. I understand that this release applies to, covers, and includes unknown, unforeseen, unanticipated and unexpected damages, losses or liabilities and consequences thereof, which result from the matters herein disclosed and known to exist.

The potential of any state, federal, local or territorial law or statute providing in substance that releases shall not extend to claims or damages which are unknown or unsuspected to exist at the time are hereby expressively waived by me. I release any photo taken of my child during this event for the use of COYFC. I am aware that if my child participates in drug or alcohol use/possession or sexual activity or refuses to comply with directions given by COYFC staff, it may result in being immediately sent home. If for any reason my child is unable to complete his/her stay on this trip, I assume full responsibility for expenses incurred for my child's return home.
